Clinical Nurse Specialist Fracture Liaison service


Location: Based at Hutt Hospital working in & across the CCHV District

Contract: Permanent, Part Time 0.5 FTE

About the Role 

Fracture Liaison services (FLS) are designed to reduce the incidence of future injury by offering appropriate assessment, education and interventions to people over the age of 50 years who experience low impact fragility fractures.  Interventions are tailored to individuals and may include medications as well as information about osteoporosis and lifestyle measures that can assist bone health and reduce risk of future fractures. 

The CCHV Fracture Liaison team has expanded to become, what is now, a district wide service with staff offering inpatient consultation and virtual or face to face clinics for those who require it. The team is small and friendly with two part-time CNS roles, a prescribing Pharmacist/Team Coordinator along with a .2fte Clinical lead/Geriatrician.  The team also includes two administrators who have a key role in screening patient reports for fragility fractures. 

The position involves the implementation of the clinical standards for Fracture Liaison Service in New Zealand, service development and evaluation to provide best practice patient care and to meet the identified Key Performance Indicators (KPI’s). 

This Clinical Nurse specialist role would suit someone keen to step into a high performing team that is making a significant contribution to fracture and frailty prevention in people aged 50 years +. 

The role includes clinical assessment for appropriate treatment, ordering relevant laboratory investigations and scans to formulate a treatment plan, as well as monitoring treatment plans initiated. 

In order to continue to develop the service we are now looking towards increasing prescribing capability within the team so this role will have the opportunity and expectation of working towards CNS prescribing.

The team is embedded in the broader Older Adults, Rehab and Allied Health service so you will have support from the Nurse Director, Older Persons, Primary and Community and access to collegiality in the form of other Senior Nurses and a wide range of professions across our multi-disciplinary teams.
