Community Dietitian / Mātanga Mātai Kai
Location: Hutt Hospital
Contract: Permanent - 16 hours per week (0.4 FTE)
ABOUT THE SERVICE
The Nutrition & Dietetic Service receives referrals for children and adults from health professionals.Those meeting our referral criteria are seen in a range of settings including within the hospital, Outpatients, GP practices and in patients’ homes.The Service keeps data regarding patient contacts.
ABOUT THE ROLE
The successful applicant will be a New Zealand Registered Dietitian who will provide dietetic care to adults (15 years old and over) as part of our community dietetic team providing care in patients’ homes and residential or supported care facilities. The reasons for referral to our community dietetic team includes Home Enteral Nutrition, Home TPN, disability, frail elderly, chronic wounds and those with neurological disorders or swallowing difficulties who have low body weight or weight loss. There will be a strong community integration focus with links to our new Te Herenga Tangata Team.
This is a permanent position of 0.4 FTE, 16 hours per week.
SKILLS AND EXPERIENCE
- NZ Registered Dietitian with current annual practicing certificate (with prescribing endorsement)
- Minimum of 2 years clinical practise
- Clinical experience applicable to allocated areas
- A commitment and understanding of the Treaty of Waitangi (and application to health) and a willingness to work positively in improving health outcomes for Maori
