Intake and Assessment Clinician (Nurse),Hutt South and Central Community Mental Health Teams.


Location:  Hutt South and Central Community Mental Health team base 40-42 Queens Drive Lower Hutt

Contract:  Full time – 40 hours per week (1 FTE), 8am – 4.30pm

About the Role

The Intake and Assessment Clinician's (Nurse) role facilitates access to mental health services through providing an initial triage and assessment service and then referring to the specialist community mental health team or to other services as appropriate.

Key priorities of the role include:

  • Triaging referrals from a variety of sources in the community.
  • Undertake face to face initial assessments. 
  • Recognise that culturally safe practices are paramount and liaise with relevant services to ensure people receive safe, high quality and equitable care.
  • Assess and escalate any crisis/urgent referrals to the appropriate crisis mental health and addiction services.
  • Works collaboratively alongside the community mental health teams and local service providers to access the most appropriate service/support for the person based on their requirements.
  • Liaise with referrers, including local GP's and community networks/NGO providers in the region to ensure people get appropriate, timely and person centred-care and feed back to them if there are any issues regarding quality of referrals

About you

We are seeking dynamic, motivated Registered Nurse who has,

  • 2-5 years of recent experience in mental health triage and risk assessment in your relevant scope.
  • An up-to-date annual practicing certificate (must have completed relevant postgrad qualifications related to mental health).
  • A strong commitment to care being provided in the least restrictive environment.
  • A passion for Mental Health assessment and enabling clients to be seen by the appropriate health professionals.

It would be advantageous if they had some experience in Intellectual disability.
