A fantastic opportunity to advance your career has arisen within the Older Persons and Rehabilitation Service (OPRS). OPRS is looking for an experienced Allied Health or Nursing professional with at least 5 years’ experience working with the multidisciplinary team in ensuring timely patient flow, transition between inpatient and community services, and effective management of complex discharges.

Our Service

The Older Persons and Rehabilitation Service (OPRS) at Hutt Hospital consists of 32 beds split into 2 wards (East and West). Our patients are cohorted between the 2 wards with sub-acute geriatric care and psychogeriatric on East and patients benefiting from a 24 hour recovery and rehabilitation model of care on West.

We provide assessment, treatment, rehabilitation and discharge planning for people of ages 16 and over who have experienced stroke, fractures and amputations or acute surgery who may require rehabilitation or reconditioning We also provide an older persons mental health inpatient service, complex discharge planning, and comprehensive geriatric assessment and treatment.

About the role

To facilitate and support patient flow and complex discharge in OPRS inpatient wards. The person in this role works closely with OPRS Associate Clinical Nurse Managers and the multidisciplinary team to ensure timely patient flow, transition between inpatient and community services, and effective management of complex discharges. This role will also provide direct clinical care based on the needs of the service. This role will also take on delegated tasks from the line manager as required.

Qualifications and experience

  • Relevant qualification in Allied Health or Nursing (essential)
  • Registered practitioner with current annual practising certificate or certification/membership of professional association if registration not applicable (essential)
  • Post Graduate qualification or working towards one
  • Comprehensive knowledge of the NZ health system and the DHB environment systems and processes
  • Knowledge of the trends and emerging issues for the service
