About the Role 

The Advancing Wellness at Home Initiative (AWHI) team is a community team at Hutt Hospital that supports people discharging from hospital following an acute medical event, neurological event or injury. The aim of the service is to improve health outcomes for patients following a hospital admission and to improve the flow of patients through the hospital. You will be working with a wide range of people and conditions in a highly dynamic environment. 

About the Team/Service/ Location 

The AWHI team has a range of professionals including Nursing, Physiotherapy, Occupational Therapy, Speech Language Therapy, Social Work and Allied Health Assistants. We also work closely with specialist doctors.

You will have access to professional support within the team as well as the relevant professional leader. 

About you 

We are looking for someone with the right clinical skills and a flexible attitude. Te Whatu Ora is commited to improving health outcomes for Māori and Pacific whānau so your commitment to Te Tiriti and health equity is a must. You will also bring excellent time and case load management, great communication and inter-personal skills. 

In order to maximise our efficiency and improve the service patient’s receive, we work in an inter-professional manner. You will need to have a holistic perspective and be prepared to use or develop your inter-professional skill set.
