Physiotherapist
Location: Hutt Hospital
Contract: Permanent, Full time, (1 FTE Rostered and Rotating)

About the Role
Are you passionate about preventing avoidable admissions and supporting people to rehabilitate well at home? In this role you will have the opportunity to support patients home early and provide their rehabilitation within their homes in the community as well as help them stay healthy and safely in the community. This role relates closely to supporting hospital flow and therefore requires to be flexible and adaptable to the changing needs of the hospital with a can-do attitude to support discharges.
About the Team/Service Location
The AWHI service is an early-supported –discharge service at Hutt Hospital that aims to support patients and their whānau at home immediately after admission to hospital. We offer three streams of rehabiliation, including rehabilitation post stroke, following an injury (under the Non-Acute Rehab contract) and with medically unwell older people presenting to ED or discharging from the Acute wards.
The AWHI team has a range of professionals including Nursing, Social Work, Speech Language Therapy, Occupational Therapy, Allied Health Assistants and Physiotherapy. We also have access to specialist Doctors for consultation.
To maximise our efficiency, we work in an Inter-professional practice model. You will need to bring a holistic perspective and readiness to skill share.
About you
We are committed to improving better health outcomes for Māori and Pacific whānau so you will bring a holistic approach to understanding people’s needs and your personal and professional commitment to health equity is a must.
You will also bring sound clinical skills, excellent time and case load management, great communication, and inter-personal skills, with at least 3 years work experience in the relevant field.
Registration with the Physiotherapy Board of New Zealand and a valid annual practicing certificate.
To enable us to offer a 7 day per week service, therapists in these roles will be rostered to work one weekend day per month with a rostered day off in the same fortnight.
