Our Social Work Service is a supportive team, committed to professional development, continuous learning and clinical improvements.The team is a mixture of inpatient and community registered social workers who readily share their expertise and knowledge.
ABOUT THE ROLE
A permanent 0.8FTE (32 hours per week) position is available to a qualified and registered social worker to work on our Acute Medical Ward. You will be a key member of the multi-disciplinary team, providing comprehensive psychosocial and risk assessments, and ensuring safe and supported patient discharges. This is an exciting, face paced, skilled social work role. You will be working with patients and their family/whanau who are experiencing varied and often complex emotional, social and physical health related needs. The role requires creative, innovative thinking and problem solving and the application of best practice of models of care.
SKILLS AND EXPERIENCE
- Registration with Social Work Registration Board and current Annual Practicing Certificate.
- Previous experience in physical or mental health.
- Excellent communication and time-management skills.
- Self-motivated and directed.
- Skilled at developing positive relationships with a variety of hospital based professionals, as well as community groups, providers and agencies.
- Full driver’s license.
