ABOUT THE ROLE
The Hutt Valley District Health Board seeks a registered social worker, with a current Annual Practicing Certificate, to work primarily on our Coronary Care Unit, General Surgical and Gynaecology, and Orthopaedic Wards. This role will cover other inpatient areas as required.
You will be a key member of the multi-disciplinary team, providing comprehensive psychosocial and risk assessments, and ensuring safe and supported patient discharges. This is an exciting, fast paced, skilled social work role. You will be working with patients and their whanau who are experiencing varied and often complex emotional, social and physical health related needs. The role requires creative, innovative thinking and problem solving and the application of best practice models of care.
This position is part time (0.6 FTE), Monday to Friday, and fixed term for six months to cover parental leave.
SKILLS AND EXPERIENCE
- Registered with Social Work Registration Board holding a current APC
- Social Work degree
- Excellent communication and interpersonal skills
- Excellent time-management and triaging skills, self-motivated and works well under pressure
- Skilled in developing relationships with a variety of hospital based health professionals in addition to community based services and primary health care providers
- Experience in a hospital setting preferable
