Team Coordinator
Location: Hutt Hospital
Contract: Permanent, Part Time 0.5 FTE

Salary: Designated Scale A – Allied, Public Health, Scientific & Technical SECA ($114,021 - 120,356) or Equivalent Nursing Scale
Advancing Wellness at Home Initiative (AWHI) are looking for a Team Coordinator to support the Team Leader to continue to expand and develop the service. The role will involve overseeing and supporting the prioritisation of clinical workloads on a daily basis as well as a variety of non-clinical tasks such as supporting with rosters, performance reviews, recruitment and service development. There will also be an expectation to support with clinical workload when needed. Ideally, this role would be combined with a clinical role within the team. This clinical aspect of this role is based in the community with an in-reach component, working in the hospital to identify patients who will benefit from the service.
About the Team/Service/ Location
The Coordinator role is based at Hutt Hospital and provides support across all AWHI streams. AWHI aims to facilitate timely hospital discharges and deliver nursing and allied health services, including re-enablement, to patients in their own homes.
The team primarily works with older adults who have been admitted to hospital following an acute medical event, stroke, or injury, supporting them to achieve their rehabilitation goals.
In this role, you will contribute to leading a collaborative, supportive, and motivated multidisciplinary team comprising Physiotherapists, Occupational Therapists, Speech-Language Therapists, Social Workers, Nurses, and Assistants.
We offer ongoing professional development opportunities, including discipline-specific training and interprofessional practice support. You will also have access to professional guidance from relevant professional leaders, as well as clinical supervision. Cultural professional supervision is available through Māori and Pacific Health teams.
About you
We are looking for an Allied Health Clinician or Registered Nurse who has current registration/APC and has recent clinical experience working with people older adults, people who have experience strokes or in rehabilitation/community settings.
The ideal candidate will have:
- a minimum of 5 years’ experience
- a current NZ driver’s license.
- excellent communication skills and enjoy supporting the development of others.
- previous leadership or service improvement activity experience would be an advantage as would an understanding of ACC.
