Kōrero mō te tūranga – About the role
The Advancing Wellness at Home Initiative (AWHI) are 7-day a week community facing service providing Early Supported Discharge (ESD) across the Capital, Coast & Hutt Valley District. The teams provide interdisciplinary assessment, treatment and rehabilitation for people up to 6 weeks after discharge from inpatient wards.
We are expanding our Early Supported Discharge Team at Hutt Hospital and are now looking for a dedicated Team Leader. Managing a team of 17 FTE this position will provide day to day leadership and management of the interdisciplinary team.
In this role you will work closely with the Team Leader for AWHI services at Capital, Coast to provide consistent models of care for early supported discharge across the District. This position is part of a wider community leadership team focused on providing care closer to home.
Mō tō mātou rōpū – About our team
We have a great team which includes Allied Health, Nursing and Rehabilitation Assistants with close links to Geriatricians.
Funded by Te Whatu Ora and ACC the service has 3 streams including; Stroke Rehabilitation, Non-Acute Rehabilitation Pathways and Acute Medical/Health of Older Persons. The team work collaboratively with inpatient wards and community services to identify suitable people who would benefit from continuing their recovery and rehabilitation at home.
Moū – About you
We are looking for an experienced Allied Health Professional or Registered Nurse with leadership and management experience. You will bring to this role a focus on clinical excellence, staff and service development and a commitment to Te Tiriti o Waitangi. Experience in supporting team members in managing clinical workloads, referrals and rostering would be of benefit.
You will hold a current Annual Practicing Certificate in your profession and relevant post-graduate qualifications are desirable.
